As colleagues in care, our interest is to support optimal treatment for individuals you refer to us for consultation as well as those that become our mutual patients. Required Medical Information
In order to provide a comprehensive evaluation of your patient we ask that you or your staff forward any pertinent medical records on to us. These records may include:
- Recent clinical notes
- Current medication list
- Discharge summaries
- Lab reports
- Cardiac and Vascular testing reports
- Operative Notes
- X-ray reports
- PFT reports
- Stress Test report
- Cardiac Catherization report
- CT, MRI or MRA
We require CT reports and actual studies to be sent via UPS or Fed-Ex to be imaged into our system--contact us for shipper account numbers.
By sending these scans to our center prior to your patient's appointment we will be able to analyze the results in advance of their visit. Thank you for your referral and your partnership in the care of your patient.

About Our Care & Services
Our surgeons perform all aspects of vascular care including a full range of open surgical and minimally invasive interventions. We also have expertise in coagulation and risk factor modifications. We work jointly with a multidisciplinary team, including Cardiologists, Radiologists and specialists in the management of hypertension and other cardiovascular risk factors.
As well as the usual clinical evaluation and surgical treatment options, we operate a noninvasive Diagnostic Vascular Ultrasound unit. Our technicians perform a full range of duplex ultrasonography for the diagnosis of carotid occlusive and venous thromboembolic disease, and doppler waveform analysis for the evaluation of arterial insufficiency.
