Varicose and Spider Veins

Patient Health Questionnaire

If your appointment is at the U-M Cardiovascular Center, we ask that you print and complete our Patient Health Questionnaire form and bring it with you on the day of your visit.

If you are a Thoracic Outlet Syndrome patient, please also complete this form.

Additional Testing

Our clinicians may need some testing done to determine the severity of your disease. Our nurses will screen the preliminary information you provide for what will be required. We may need x-rays or films from your referring doctor, or some ultrasound studies that we will likely obtain here at the hospital.

We participate with most insurance plans such as Blue Cross/Blue Shield, Medicaid, Medicare, Premier Care, TRICARE Standard, and numerous managed care plans and programs.
