![]() Making An AppointmentTo schedule with a vascular surgeon at our Cardiovascular Center clinic, please call us at (888) 287-1082. Our clinicians may need some testing done to determine the severity of your disease. Our nurses will screen the preliminary information you provide for what will be required. We may need x-rays or films from your referring doctor, or some ultrasound studies that we will likely obtain here at the hospital. We will ask you to fax certain information to (734) 232-4480, and bring other data with you. Depending on your insurance, you may need a referral from your primary care physician. Medicare and traditional Blue Cross/Blue Shield patients will not, but most others will. Please contact your referring doctor to obtain that authorization if your carrier requires it. That can be faxed to the same number. At our Cardiovascular Center clinic, we have an option to schedule you with one of our multidisciplinary faculty also. Particularly if your condition is not surgical, we may suggest this option. If you have a venous condition, especially varicose veins or spider veins or less life-threatening conditions in your legs, perhaps the surgeons at our Livonia Vein Center can provide your care. They will answer any questions you have about this option if you contact them at (734) 432-7662. To assist in making your first appointment go more smoothly, we do have our patient profile form available to you on line if you would like to complete it in advance.
