About the Clinic

Children's Intestinal Rehabilitation Program

The Division of Pediatric Gastroenterology, Section of Pediatric Surgery, Parenteral and Enteral Services, Nursing and Department of Pharmacy Services all participate in a clinic specializing in short bowel syndrome intestinal failure. This clinic focuses on coordinating the nutritional and gastrointestinal care of infants and children who suffer from short bowel syndrome or intestinal failure from either malabsorption or dysmotility. We provide for routine monitoring of parenteral and enteral nutrition, work closely with patients in making transitions off TPN, and carefully select patients who may benefit from bowel lengthening procedures.

Who is Eligible to be Treated in this Clinic?

  • Patients with difficulty maintaining weight and hydration without receiving TPN or IV hydration.
  • Children diagnosed with Short Bowel Syndrome.
  • Children diagnosed with Intestinal Dysmotility.
  • Children with complex surgical problems which prevent them from taking adequate nutritional intake.
  • Age range is from newborn to 18 years of age.
  • Patients who require specialized tube feedings for fluid or nutrients.
  • Patients who need to reduce diarrhea and control volume of stool output.

Intestinal Failure Clinic Services

The outpatient clinic is located on the seventh floor of Mott Children's Hospital in the Pediatric Surgery Clinics (room F7784). Children (Ages: newborn to 18 years) with diverse and challenging nutritional needs related to a variety of gastrointestinal problems are seen on a minimum of once a month. Patients seen in the outpatient clinic may suffer from short bowel syndrome, complex malabsorptive conditions, radiation enteritis, pseudo-obstruction and motility disorders. The Intestinal Failure Clinic outpatient clinic provides consultation to primary care providers and specialist physicians who are caring for complex patients with intestinal failure. Following comprehensive multi-disciplinary examinations, a plan of care is provided for both the patient and the referring physician.

    Nutritional Therapy

    All patients will undergo a complete nutritional analysis. Our physicians and dietitians work closely to determine recommendations regarding optimal TPN management, fluid and electrolyte management and special diet designed to meet the patient's specific needs. These recommendations are provided to the patient and referring physician to help achieve desired outcomes. Goals are to increase weight, decrease stool output, improve hydrations status and improve nutrient deficiencies.

    Surgical Therapy

    A surgeon is available for consultation at clinic and will make recommendations specific to the individual need and clinical presentation. Surgical procedures performed at include bowel lengthening either by a standard Bianchi or STEP procedure and bowel tapering. Additionally, routine gastrointestinal surgeries, including feeding tube placement and management, and antireflux procedures are provided.
