If you are unable to keep a clinic or surgery appointment please notify us at least 7 days in advance of the appointment. If our department is not notified within a timely manner it may affect patient care or the ability to reschedule the appointment. Not only is this courteous to our department, but also allows other patients the ability to be seen sooner.
New Patient Visit
These visits are for evaluation only and do not in any way indicate a guarantee of care or insurance coverage.
Once a request for surgery has been received from the attending physician, a prior authorization will be requested for the surgical procedures required. A typical turnaround time from submitted request to scheduling of surgery is 30 days. This is adjusted based on medical urgency. Since this is a department that deals with urgent issues, an elective case will be secondary to life threatening cases.
In most cases you need a referral. To initiate a referral, please contact your primary care physician and have the referral coordinator issue an insurance referral and fax it to our clinic at (734) 936-6290.
If you have an HMO health provider please allow at least 30 days to receive an insurance authorization for surgery. When we receive an authorization we will contact you to schedule a surgery date. If you receive an insurance approval/denial please notify our office so that we may discuss an appropriate action.
Insurance companies that typically need an insurance referral and/or authorization:

Coverage of Surgical Procedures
Although our scheduling office will prior authorize medically necessary procedures, you are responsible for knowing your insurance and dental benefits including co-pays, deductibles, exclusions, and non-covered procedures.
Extractions are not expected to be covered by medical insurance. Implants are not expected to be covered by medical insurance.
Elective surgery may not be covered and cosmetic surgery is not covered.
Day Before/Day of Surgery
Once your surgery date is established, please be aware that an arrival time will not be provided by our scheduling office as this is determined by the Pre-op Care Unit. You will receive a phone call the day before your surgery date to notify you of your required arrival time.
The operating room preoperative department calls patients 11am - 5pm, the day prior to surgery, to notify patients of their time of arrival as well as any necessary precautions or preoperative instructions. If you do not receive a call by 5pm the day before surgery you may contact the necessary departments below.
